Configuração .HTACESS URL

Olá alguem pode me dar umas dicas de como configurar corretamente o arquivo HTACESS e modificar a URL principal do site que estou desenvolvendo?

1- ao digitar o endereço <!-- w -->[url=http://www.luciancairuz.com" onclick="window.open(this.href);return false;]www.luciancairuz.com[/url]<!-- w --> aparece na barra de endereços: http://www.lucianocairuz.com/index.php/en/
gostaria que aparecesse apenas [url=http://www.lucianocairuz.com" onclick="window.open(this.href);return false;]http://www.lucianocairuz.com[/url] na pagina principal.

2- Ao clicar no menu palestras, ao acessar este artigo o site aponta para a url: http://www.lucianocairuz.com/index.php/ ... ano-2.html

porque tudo isso? gostaria apenas que ficasse: http://www.lucianocairuz.com/drluciano-2.html

é possivel? Como proceder?

li em alguns artigos que isto deveria ser feito no arquivo .HTACESS como configurar corretamente. no meu diretorio raiz existem dois arquivos. HTACESS.txt e outro .HTACESS
